Ionitch understands that he ruined his life with his own hands, but he can not do anything about it.
Thus, Chekhov is trying to show how some people can influence others. Moreover, this influence is not always positive. As we see, Startsev, who opposed society, became his typical representative. He was swallowed by the philistine vulgarity that reigns in the county. From a talented doctor, Dmitry turned into a boring and lazy Ionych.
